    Concussion Acknowledgement Form

    April 1st

    Hello, CROCS!!

    The Georgia Legislature passed the Georgia Return to Play Act of 2013, which requires parents and athletes to read and sign information regarding concussions and how to recognize them. We have uploaded the form that all parents and swimmers will need to sign before practices begin. Please print, read, sign, and bring your completed forms to registration on April 8. We will have extra forms on hand at registration. Also, the 2014 Registration Forms have been uploaded. Both forms are available in the links tab on the left side of the home page.

    Julia Hodgson Assistant Head Coach
